At N4, ことにする meant deciding something. ことにしている is a decision that has become a habit: 毎朝ジョギングすることにしている, "I make a point of jogging every morning".
It's a personal rule that you keep to. With the ない-form, it's something you've decided not to do: 夜十時以降は何も食べないことにしている.
Compare ことになっている, which is a rule set by someone else (a school, a company, a schedule). ことにしている is always your own choice.
It's close to ようにしている, which is more about trying to do something. ことにしている is firmer: you've made it a rule.
