を皮切りに names the first in a series of events that then spread or continue: 東京公演を皮切りに、全国ツアーが始まる, "the national tour kicks off with a show in Tokyo".

The second half is a series or expansion: a tour, a run of complaints, new branches opening, a wave of people speaking out. It's common in news, business and entertainment.

Compare をはじめ (N3), "including, starting with", which lists representative members of a group that already exists. を皮切りに is about time: this was the first, and more followed.

皮切り originally meant the first moxibustion treatment on the skin, the first "cut".
